YNF believes that ever life is worth saving and in need is guidance. That’s is why when the staff first engage the client they will go through an internal assessment and tracking in order to track they progress.

They will cover a comprehensive range of potential client’s needs, which includes medical, mental, substance abuse, financial, legal and housing status. Some services will be addressed onsite and other will be referral to outside partnerships. The referrals will be geared towards the appropriate medical, mental health, and specialized treatment based programs.

Continuity of care will be a coordination of the different services provided.

They will assistance the individual with obtaining Public Assistance, Medicaid, SSI and Veterans benefits if they qualify and follow-up paperwork.
The clinical team will make sure every client has a roof over his head and an active PA Case. They will meet with the clients to help them understand the importance of maintaining an open PA as it relates to housing. They will elaborate in more details regarding their security deposit, broker’s fee and furniture funds when they are linked to housing.

These services will begin at the time of intake and continue until the resident leaves the program.
